This position as a Care Advocate utilizes social work skills, case management, and excellent networking/outreach skills with other community resources. The Advocate ensures that seniors and the disabled are provided with assistance and access to resources so that they may better manage their health issues, maximize their independence, and enjoy an enriched quality of life.
